Athletics Forms 2025-26
Three forms needed/student-athlete
Each St. Ignatius student planning to participate in a St. Ignatius school sport must have on file three forms in the office: 1) their physician signed medical release form; 2)Athletics Emergency form; and 3)signed acknowledgement form for Concussion, Sudden Cardiac Arrest, and Opiod fact sheets. The forms are due prior to tryouts (Girls Volleyball and JV & Varsity basketball) or the first practice for all other sports.
Forms may be dropped off in the school office OR scanned and emailed to Lisa and Carol at lschneider@stignatiussacschool.org and cnelson@stignatiussacschool.org
The physician signed medical release form is valid for the entire school year and must be signed after June 15, 2025. Most physicians will sign this form as long as they have seen the patient in the past year.
The Athletics Emergency form is completed for each sport in which the student participates.
Earn one Athletic hour for turning in all three required forms by the first day of school! Even if your child is not participating in a Fall sport, you can still turn in the forms ahead of their season (Little Dribbler encouraged to turn in their forms at this time too!) to earn the Athletic hour.
St. Ignatius Soccer is not a school sponsored sport and does not require the forms attached.
Cost to participate is $100/sport (Golf is $200) and is billed through FACTS.
Fall sports: Girls Volleyball (5th – 8th grade): Co-ed Cross Country (5th – 8th grade); Boys Flag Football (7th & 8th grade)
Winter sports: Basketball (1st – 8th grade)
Spring sports: Girls Flag Football (7th & 8th grade); Co-ed Track (5th – 8th grade); Boys Volleyball (6th – 8th grade); Co-ed Golf (5th – 8th grade)
