We had a technical error and the signature line and date for ”Authorization for Consent of Treatment of Minor 2017-2018” was not available in the Schoolspeak profiles for your children until yesterday.   

Please log-in to Schoolspeak.com and sign and date this very important medical permission statement as soon as possible so we can print our Emergency Cards for the coming school year.  We have moved this field near the top of the profiles, immediately under the phone numbers.
